Paano Mag-Apply Ng Financial Assistance For Medical Purposes Sa PCSO

  Ang Medical Access Program (MAP) ng PCSO ang nakatutok sa pagbibigay ng financial assistance sa mga mamamayang hindi sapat ang pambayad sa mga gastusing medikal. Kaagapay nila rito ang government at private hospitals, pati na ang health facilities, medicine retailers, at iba pang partners.

  Ngayong panahon ng COVID-19 pandemic, patuloy pa rin ang MAP para sa National Capital Region (NCR) application. Pero nagbigay na ng abiso ang PCSO na hindi na sila tumatanggap ng walk-in application para makaiwas sa paglaganap ng coronavirus infection. Dinadaan na lang ang proseso sa pamamagitan ng email.

  Alamin ang medical coverage at mga requirements ng bawat isa

  Bisitahin ang website ng ahensya. I-click ang “Programs and Services” at piliin ang “CAD Programs and Services.” Dito mo malalaman ang available medical coverage at, kung pasok ka sa listahan, isunod ang PCSO requirements for financial assistance.

  Sa ilalim ng Confinement medical coverage, ihanda ang mga sumusunod:

  • Original copy of the Final Statement of Account/Latest Hospital bill with printed name duly signed by the Billing officer/Credit Supervisor with PhilHealth, Senior Citizen, HMO, MSS/Discounts deductions
  • Kung discharged na sa ospital: Validly-executed Promissory Note duly signed by the hospital representative or Certification with remaining balance from the hospital
  • Para sa kasong medico-legal: Copy of the Vehicular/Police Report

  Sa ilalim ng Chemotherapy medical coverage, ihanda ang mga sumusunod:

  • Original prescription with printed full name, signature and license number of oncologist/ attending physician
  • Original copy of treatment protocol with signature, name and license number of oncologist/attending physician
  • Photocopy of Surgical/Histopathology/Biopsy Result
  • Index Card for patients with previous assistance

  Sa ilalim ng Dialysis (Hemo/PD/Erythropoietin) medical coverage, ihanda ang mga sumusunod:

  • Acceptance Letter from the Dialysis Center/ Hospital accepting guarantee letter from PCSO
  • Official Quotation from the Dialysis Center/ Hospital (for dialysis)
  • Prescription with printed full name, signature and license number of the attending physician (for Erythropoietin)
  • Photocopy of relevant laboratory result/s within the last three (3) months
  • Index Card for patients with previous assistance
  • Para sa Philhealth members: Copy of Member Data Record at Certification on the number of benefits availed

  Sa ilalim ng Medicines (Hemophilia and Post-transplant) medical coverage, ihanda ang mga sumusunod:

  Sagutan ang application form

  Sa PCSO website pa rin, i-download ang IMAP application form at sagutan ito. Pagkatapos, i-scan o kunan ng litrato ang nasagutan mo ng application form. Kakailanganin mo iyan sa pagsusumite ng application form.

  Ipadala ang application form at requirements sa pamamagitan ng email

  Bukod sa application form at scanned/photographed copy nito, kailangan mong mag-attach ng documents base sa requirements ng napapalooban mong medical coverage. Tandaan ang babalang ito ng PCSO: “Only applications with complete documents shall be processed on a first come-first served basis.”

  Pagkatapos mong mag-email, makakatanggap ka ng acknowledgement at status of request sa email address na inilagay mo sa iyong application form. Kasunod nito ang claim slip at guarantee letter. Gumawa ka ng printout ng claim slip at guarantee letter.

  Sa loob ng 30 days simula nang makuha mo ang claim slip at guarantee letter, isumite ang printed copies sa ospital o di kaya health facility at medicine retailer kalakip ang requirements ng medical coverage kung saan ka humihingi ng tulong.

  Ayon sa ahensya, bukas ang pagpapasa ng request for medical assistance mula Monday hanggang Friday. Magsisimula ito ng 10 a.m. at matatapos hanggang marating ang “allowable daily budget for MAP NCR.” Kaya mainam kung sisikapin na kumpletuhin kaagad ang PCSO requirements for financial assistance para sa medical purposes.
